Background:The ASPOC Project (ASPPOC, AntibioticSurveillance Project on Perioperative Chemoprophylaxis)is a multicentric study carried out in European countries,aiming at establishing an evidence-based perioperative che-moprophylaxis.Objectives:The aim of this study, which is part ofASPPOC, was to check the conformance to the guidelinesfor perioperative chemoprophylaxis in Orthopedics Surgery,and to trace out changes that must be made in Europeancountries to improve its quality.Methods:3 European countries participated in the study:Greece, Italy and Croatia. The conformance to the recom-mended guidelines and the quality of perioperative prophy-laxis were checked by use of the same questionnaire ineach country. The following criteria of quality werechecked: application of perioperative prophylaxis, durationand time of initiation of prophylaxis, and the kind of antibiotic used. Results:Parenteral chemoprophylaxis was applied inalmost all operations, was initiated during preinduction toanesthesia and its duration exceeded the maximum recom-mended time of 24 hours in most cases in the 3 countries.2nd generation cephalosporins, vancomycin, metronidazoleand amikacin were used in Greece. A 1st generation cepha-losporin, cefazolin, was used in Croatia. In Italy, teicopla-nin, vancomycin, cefazolin and amoxycillin clavulanatewere used. 3rd generation cephalosporins were used inCroatia and mainly in Greece.Conclusions:The conformance to the guidelines and thequality of perioperative prophylaxis were not satisfactory.Dynamic changes are required to improve the quality andestablish an evidence-based perioperative prophylaxis in orthopedics surgery.
