TP53 screening could be useful in a patient stratification algorithm for adult acute lymphoblastic leukemia (ALL). TP53 mutations are more frequent in T-ALL than in B-ALL and can be detected in 14% of cases negative for recurrent fusion genes, regardless of lineage derivation. Given the negative influence on response to induction chemotherapy, we suggest that TP53 status should be investigated at diagnosis particularly in patients negative for recurrent fusion genes, where the genetic-based prognostic stratification is still limited.
