TOSCA-IT is an example of how diffi cult large independent intervention trials are to do because of scarce funding from governments and independent bodies, which results in financial resources at risk of not covering costs, particularly if the study duration exceeds the original expectations because of lengthy recruitment and a decrease in cardiovascular mortality and morbidity, as occurred in the past few years. The major risk for this study is its premature ending if additional funds from AIFA or other sources do not become available. We believe such an outcome would show a failure of independent research and a formal abdication to industry for the generation of evidence-based medicine.

Abstract
