Current treatment of HSPN is primarily based on indirect evidence and on observational studies. Similarly to a previously published analysis, the meta-analyses performed by Chartapisak and co-workers highlight the lack of informative evidence. Despite the fact that HSPN is a frequent glomerular disease in children, we still don't know which patients to treat and how best to treat them. While no evidence exists supporting preventive treatments, it appears reasonable to propose immunosuppressive regimens to patients with severe HSPN, until new data are available. The indications for treating milder cases, particularly in the absence of overt proteinuria, are more disputable.
