Renal transplant recipients (RTR) present many skin diseases due both to the side effects of immunosuppressive drugs and to the drug induced chronic immunosuppression. Many of the skin diseases have only aesthetic significance, however HPV infections and skin cancers have many important consequences on the conditions of the patients. Genotype, environmental factors, generalized immunosuppression and cutaneous UV induced immunosuppression are the favouring factors of HPV infections. In the etiopathogenesis of skin cancer, in RTR, HPV infection and the mutagenic action of the immunosuppressive drug azathioprine, play an important part. We present three cases: in the first we show the correlation existing between photoexposition and HPV infection, in the second and the third the neoplastic transformation of verrucous lesions. Our work demonstrates that RTR need and accurate dermatologic follow up to prevent onset of HPV infections and skin cancers and to treat all the high risk lesions early.
