Polycystic ovary syndrome (PCOS) is a very common disorder of women in the reproductive age. This condition has a wide range of medical outcomes, including implications of increased risk of cardiovascular disease later in life. Despite the high prevalence and the relevant medical impact of the syndrome, its pathogenesis remains largely unknown. The understanding of the pathogenetic mechanisms of PCOS is hampered by the existence of intricate relationships among the characteristic features of the syndrome. In this labyrinth many routes lead back to the starting point, making it very hard to establish what is primary and what is secondary. Whether we are close to getting out of the labyrinth or we are coming back to the starting point is still an unsolved doubt. Nevertheless, new therapeutical approaches to the syndrome, potentially active against the pathogenetic mechanisms, are, for the first time, promising.
