The resistance to aminoglycosides is increasing and thecombination with vancomycin (V) is useful in orthopaedic infections.Gentamicin (G) and V are known to have a potential nephrotoxic effect, especially when administered in combination. Toxic serum levels are >10 mg/l for G and >60 mg/l for V, respectively.Industrial spacers loaded with gentamicin and vancomycin (1.9% + 1.9%) were studied in an international multicenter clinical trial (Italy, Spain, Switzerland). The primary endpoint was to assess their safety when implanted in patients with prosthetic joint infection (two-stage revision), also when the devices are utilized along with the systemic antimicrobial therapy. The secondary endpoints were related to the mechanical safety and to the functional effectiveness (joint mobility andre-implantation).
