Sjogren's syndrome is a wide and complex clinical entity, which may be primary or related to other rheumatological disorders. The causes, the pathogenesis and the diagnosis of the syndrome have been widely discussed but remain somewhat controversial. We have tried to depict a current outline of the problems concerning the syndrome, with special regard to the possible pathogenetic role of a lot of viruses (HTLV-I, Epstein-Barr virus and others, including defective retrovirus). Besides, we report a short review about a new interesting aspect, that is the relationship between Sjogren's syndrome and lymphomas, particularly of the MALT-type. There are some evidences that a vital infection could be responsible for the possible evolution into malignancy. The data acquired in the last few years are likely to disclose weigthy changes in the treatment of Sjogren's syndrome. Moreover, some acquisitions concerning this disease could be useful with regard to other rheumatological disorders.
