The inappropriate use of proton pump inhibitors (PPIs) is common in internal medicine wards. Previous results from the REPOSI (The Registro Politerapie SIMI) registry reported a high prevalence of PPI use, with fewer than half of these prescriptions deemed appropriate according to national and international guidelines. Recently, we also found that unnecessary PPI prescriptions were associated with a nearly 60% increased risk of 1-year mortality and a 27% higher risk of 1-year re-hospitalization compared to non-users. Common predictors of inappropriate PPI use include polypharmacy, multiple comorbidities, and the use of corticosteroids and anticoagulants, which do not independently require gastroprotection.
