An accurate characterization of mediastinal lesions is still a challange in prenatal diagnosis. Mediastinal cysts are congenital, benign conditions and encompass a wide spectrum of anomalies with no specific ultrasound (US) features but variable clinical implications. Pericardial cysts (PCs) are very rare lesions that mostly resolve spontaneously. Only few cases have been described during prenatal life. In this report we describe the first case of a rare complication of a fetal pericardial cyst diagnosed by US and magnetic resonance imaging (MRI), and conservatively managed with no impact on perinatal health.
