BACKGROUND Negli ultimi due decenni è stata rivolta una crescente attenzione al tema della valutazione della qualità dei servizi sanitari e sociali. Capire come i sistemi di salute mentale sono organizzati e come possono migliorare è fondamentale, dato l’impatto che i disturbi psichiatrici hanno sulla morbilità della popolazione, come misurato dal Global Burden of Disease. Nell’ultimo decennio, con la diffusione del concetto di ‘recovery’, il ruolo dei servizi sociali nel supportare la riabilitazione di persone con problemi psichiatrici ha assunto crescente rilevanza. Tuttavia, il contributo scientifico delle scienze sociali in questo settore è molto limitato rispetto al volume di studi prodotto dalla disciplina medica. OBIETTIVI (1) Identificare/sviluppare nuovi indicatori di qualità delle cure psichiatriche e sociali fornite a persone con problemi di salute mentale; (2) Valutare la qualità della vita lavorativa tra il personale dei servizi psichiatrici e indagare come questa possa incidere sulla qualità delle cure fornite; (3) Studiare la qualità delle cure psichiatriche e sociali attraverso uno misura della continuità delle cure secondo il punto di vista degli utenti; (4) Studiare la qualità delle cure psichiatriche e sociali attraverso uno misura della qualità della vita; (5) Valutare le associazioni tra la qualità della vita, la continuità delle cure, e l’utilizzazione dei servizi psichiatrici e sociali. METODI Gli obiettivi sono stati sviluppati all'interno di due progetti di ricerca: un progetto finanziato dall’Unione Europea - REFINEMENT (Research on FINancing systems’ Effect on the quality of MENTal health care) - e uno studio multicentrico italiano. È stato utilizzato uno strumento di mappatura dei servizi (REMAST) al fine di standardizzare la classificazione dei servizi psichiatrici e sociali in otto paesi europei. È stata realizzata una revisione della letteratura sugli indicatori di qualità dei servizi psichiatrici e sociali. All’interno dello studio multicentrico italiano sono state adottate misure a livello sia di staff che di utenti. RISULTATI Il numero totale dei servizi di salute mentale mappati nelle otto aree di studio del progetto REFINEMENT è di 1270. A seguito della revisione della letteratura, è stato sviluppato e testato in otto paesi europei uno strumento per la valutazione della qualità delle cure. Lo studio multicentrico italiano ha coinvolto 443 professionisti e 220 utenti. L’indagine sulla qualità della vita lavorativa ha dimostrato che i livelli di soddisfazione, di compassion fatigue e burnout sono influenzati sia da caratteristiche personali che organizzative. Gli utenti hanno riportato una generale soddisfazione con varie dimensioni della continuità delle cure. Gli utenti ottenevano outcome positivi dall’utilizzazione dei servizi sociali. Inoltre, una migliore esperienza di continuità delle cure era associata a una migliore qualità di vita. CONCLUSIONI Legislatori, esperti e responsabili dei servizi potranno utilizzare gli strumenti prodotti nel progetto REFINEMENT per comprendere la complessità dei sistemi di cura per la salute mentale. I risultati dell’indagine sulla qualità della vita lavorativa permettono di valutare gli elementi che incidono sulla qualità della vita lavorativa dello staff e come questa possa incidere sulla qualità delle cure fornite. Infine, lo studio multicentrico ha permesso di raccogliere informazioni rilevanti sugli aspetti ritenuti prioritari dagli utenti dei servizi psichiatrici.
BACKGROUND The last two decades have witnessed an increasing emphasis on performance and quality assessment of health and social care services. Understanding how mental health systems are designed and how they can best function is critical, given the impact of poor mental health on the morbidity of the population, as measured by the WHO Global Burden of Disease. In the last decade, with the spread of the concept of ‘recovery-oriented’ approaches in mental health, the relevance of social care provided to users with mental health problems has become crucial. However, the contribution of social care research is very limited compared with the volume of research undertaken within medical psychiatry. OBJECTIVES (1) To identify existing and develop new indicators of quality of mental health and social care provided to people with mental health problems; (2) To assess the quality of working life amongst mental health staff and investigate how this might affect the quality of care; (3) To investigate and describe the quality of mental health and social care using a measure of service users’ continuity of care; (4) To investigate and describe the quality of social care services using a measure of social care-related quality of life; and (5) To assess the relationships between social care-related quality of life, continuity of care, and utilization of mental health and social care services. METHODS The objectives were developed within two research projects: an EU funded project - REFINEMENT (Research on FINancing systems’ Effect on the quality of MENTal health care) - and an Italian multicentre study. The REFINEMENT Mapping Services Tool was used aiming at standardizing description and classification of mental health and social services. A literature review on quality indicators of mental health and social care was conducted. The Multicentre study included measures both at staff and service user level. RESULTS The total number of mental health services mapped in the eight REFINEMENT study areas was 1270. As a result of the literature review, a tool for quality assessment was developed and piloted in eight European countries. The Italian multicentre study involved 443 mental health professionals and 220 service users. The survey on quality of working life showed that both personal and organizational characteristics influence the levels of satisfaction, compassion fatigue and burnout among mental health staff. Service users reported overall satisfaction with several dimensions of continuity of care. They seemed to obtain positive outcomes by using social care services. Moreover, the better the experience of continuity was, the better a person’s social care-related quality of life. CONCLUSIONS The REFINEMENT tools are now available to policy makers, researchers and service providers to systematically assess mental health systems. The findings from the staff survey might be useful to managers and administrators to appraise elements affecting their staff’s quality of working life and how they might affect the quality of care. Finally, the study has collected important information on what aspects of mental health services are perceived as important by service users, which may be useful for policy makers and service providers.
