Le infezioni da virus dell’epatite B e C sono tra le cause più diffuse di malattia cronica del fegato e portano ad una serie di complicanze come la cirrosi e l'epatocarcinoma. Quest'ultimo è correlato a fattori come l'età, la viremia, la gravità della malattia epatica e a fattori geografici ed il rischio di patologia tumorale aumenta se è presente anche la cirrosi. L'infezione cronica del fegato scatena una serie di risposte e attiva vari meccanismi che, nel lungo periodo di tempo in cui la malattia agisce, portano alla fibrosi, che può essere interpretata come vero e proprio indicatore dello stato clinico-funzionale dell'organo. La prognosi e la gestione clinica dei pazienti affetti da malattia cronica di fegato sono largamente influenzate dall’accumulo di fibrosi e dalla sua progressione nel tempo e le stesse scelte terapeutiche sono condizionate da questo parametro. Un’identificazione precoce e affidabile delle lesioni tanto da permettere un rapido e corretto approccio terapeutico rappresenta il principale obiettivo della moderna diagnostica per immagini. Da anni la biopsia epatica rappresenta l’unico strumento di valutazione della fibrosi. Tuttavia la biopsia è una procedura invasiva che talora può essere dolorosa ed indurre complicanze e ciò può scoraggiare alcuni a sottoporsi a questa valutazione In questo contesto, si inserisce tra le tecniche di Imaging la misurazione della fibrosi epatica mediante la fibroelastometria ad impulsi (Fibroscan) che consente di stabilire in modo non invasivo l’entità della fibrosi, permettendo grazie alla sua accuratezza di seguirne l’evoluzione nel tempo. L’esame Fibroscan consiste nell’applicare un piccolo impulso meccanico alla cute appoggiando una sonda ecografica modificata a livello di uno spazio intercostale sovrastante il parenchima epatico al fianco destro. Lo strumento analizza, tramite un fascio di ultrasuoni, la velocità dell’onda meccanica indotta e calcola un indice numerico espresso in ChiloPascal (KPa), valutando la fibrosi del fegato. La velocità di propagazione di un’onda meccanica che attraversa il fegato è proporzionale alla rigidità dell’organo: quanto più il fegato è rigido tanto più velocemente si propaga l’onda. Nel nostro studio iI Fibroscan ed il marcatore biochimico FIB4 sono esami utili nell’ identificare soprattutto soggetti con cirrosi senza eseguire biopsia epatica. Tuttavia per la valutazione clinica della maggior parte dei pazienti con fibrosi lieve moderata la biopsia rimane la tecnica di elezione.

In this context, there is between the imaging techniques measurement of liver fibrosis by fibroelastometria pulse (Fibroscan) that allows to determine non-invasively the extent of fibrosis and allows to see by its accuracy the fibrosis evolution over time . The examination Fibroscan involves applying a small mechanical impulse to the skin by placing a modified ultrasound probe at the level of an intercostal space above the liver parenchyma on the right side. The tool scans, through a beam of ultrasonic waves, induced mechanical wave velocity and calculates a numerical index, expressed in kilopascals (kPa), assessing liver fibrosis. The speed of propagation of mechanical waves passing through the liver is proportional to the rigidity of the organ: the more rigid the liver is the faster the wave propagates. In our study Fibroscan and the biochemical marker FIB4 are useful tests to identify especially patients with cirrhosis without performing a liver biopsy. However, for the clinical evaluation of most patients with mild-moderate fibrosis, biopsy remains the technique of choice.
