Il nucleo sub-talamico (STN) rappresenta ad oggi il bersaglio ottimale per la sitmolazione cerebrale profonda (DBS)nella malattia di Parkinson complicata. L'utilizzo della RM cerebrale a campo ultra-alto quale metodica di suo reperaggio è raramente descritta in letteratura. Obbiettivi di questo studio sono di definire l'errore medio di centramento del nucleo utilizzando una RM 3 Tesla non stereotassica fusa con una RM cerebrale ad 1 Tesla stereotassica e di verificare l'impatto clinico della metodica. . MATERIALI E METODI. 20 pazienti con malattia di Parkinson complicata sono stati sottoposti a STN-DBS presso il nsotro Dipartimento M:F=12:8 Età media 56 anni, durata di malattia media 11,6 anni. Attraverso due differenti modalità di targeting sono stati identificati due gruppi:Gruppo A (6 pazienti, RM a 1 tesla stereotassica – targettamento indiretto e Gruppo B (14 pazienti, RM a 3T non-stereotassica fusa con RM a 1T stereotassica. In tutti i casi si è utilizzato l'ausilio della neurofisiologia intraoperatoria ed il controllo del posizionamento dell'elettrodo con fluoroscopia in sala operatoria. RISULTATI. Tutti i Pazienti presentavano un adeguato follow-up (minimo 6 mesi- medio 20 mesi) La riduzione media del punteggio di UPDRS III (med-off/stim.-on vs. preoperative med-off) è stata del 69% nel Gruppo A e del 74% nel Groppo B (p=0.01487, chi-square). La riduzione della terapia farmacologica antiparkinsoniana è stata del 66% nel Gruppo A e del 75% nel Gruppo B (p=0.58). La traccia centrale pre-operatoria (corrispondente al targettamento ideal) è risultata la più efficace dal punto di vista clinico in 2/12 elettrodi nel Gruppo A e 21/28 nel Gruppo B (p<0.001). In tutti i Pazienti è stato calcolato, sulla base di una Tac cerebrale stereotassica a strato sottile post-operatoia, l'errore medio di reperaggio del nucleo nei tre piani dello spazio. X: 0.8 mm +/- 1 SD Y: 0.95 mm +/- 2 SD Z*: 1-2 mm +/- 2 SD CONCLUSIONI: La RM cerebrale a 3 Tesla sembra rappresentare uno strumento di estrema utilità nel reperaggio del STN nella DBS. La sua distorsione magnetica sembra ridotta dalla fusione con una RM cerebrale stereotassica a campo convenzionale (1 Tesla) La neurofisiologica intraoperatoria rimane fondamentale nel determinare la profondità dell'elettrodo (coordinata Z) e nel prevenire effetti collaterali di malposizionamento.

It is based on the direct identification of STN on MR T2 weighted images. The nuleus is anterior and lateral to red nucleus anterior border on coronal images. As discussed during the II year relation, in this study it is used a third, new technique: a non-stereotactic 3T MRI fused with a 1T stereotactic MRI (routinely adopted for indirect targeting) in order to reduce 3T peripheral magnetic field distortion. MATHERIALS AND METHODS. 20 patients with advanced PD underwent STN-DBS at our center. (M:F=12:; mean age 56y; mean disease duration 11,6 yrs). According to the different targeting methods were divided into two groups: Group A (6 patients, 1T stereotactic MRI alone – indirect targeting) and Group B (14 patients, 3T non-stereotactic MRI fused with 1T stereotactic MRI- direct targeting). Intraoperative neurophysiology - microrecording exploration (3-5 tracks) and macrostimulation - were always performed. Intraoperative leads position was checked with direct fluoroscopy. RESULTS. All patients had an adequate follow-up patients have an adequate follow-up of minimum 6 months (mean 20 months). . Magnetic field distortion is reducede by fusion with a stereotactic 1T MRI.
